Description

OnTheDOT aims to improve access to dialysis for those living along the Swiss-Italian border, with the goal of reducing mortality and comorbidities. The initiative seeks to decentralize hospital care by shifting it toward local community centers and patients’ homes. This is achieved by standardizing clinical data collection in peripheral facilities and introducing digital decision-support systems to personalize therapies. Furthermore, the project includes a cross-border telemedicine pilot study, training activities for healthcare personnel, and the definition of protocols for the home care of stable or terminally ill patients.
Funded by the Interreg VI-A Italy-Switzerland 2021-2027 cross-border cooperation program (co-funded by the European Union).
